We welcome our community members and medical professionals to an evening of light refreshments and an introduction to our new suite, which houses our Supportive Care Center as well as Marin General Hospital’s Speech & Audiology Center.
Join us to learn about this innovative new resource in Marin for people with chronic or serious illnesses, and their caregivers.
-Complex Care case management and resource connection for people dealing with multiple chronic conditions.
-Palliative Care, which focuses on enhancing quality of life by helping people address their pain, symptoms, and/or medication side effects.
-Advance Care Planning to assist with discussions about your preferences for care, resources for long-term care, medical forms, and care transitions.
